*진단된 환자는 첫 내원 시 바로 치료를 시작하는 것이 좋다.

*3-7일 후 반복할 수 있으며, 필요하다면 1주일 후에 반복할 수 있다.

*가려움증은 치료 후에도 몇 일 또는 몇 주 동안 지속될 수 있다.

*증상 호전이 없을 경우에는 1주 후 다시 치료 할 수 있다.

*또한 우리나라에서는 crotamiton 연고도 판매되어 사용되고 있다. (근거수준 4)

*병변부위를 씻고 이 박멸제제 (크림, 로션 또는 샴푸)를 지시대로 바른다. 

*이환된 음모나 몸의 모든 털이 있는 부분에 박멸제제를 바른다.

*콧수염 또는 턱수염에 감염이 있는 경우 동일하게 박멸제제를 바른다.

 A   Permethrin 1% 로션을 이환된 털에 바른 후 10분 후에 씻어낸다.
 A   0.33% pyrethrin-piperonyl butoxide 샴푸를 이환된 털에 바른 후 10분 후에 씻어낸다.
 A   Lindane 1% 로션 또는 크림을 이환된 털에 바른 후 4분 후에 씻어낸다.
 D   Crotamiton 10% 연고를 이환된 털에 바른다.

===치료약물의 주의 사항===

*Gamma benzene hexachloride (lindane)는 신경독성을 일으킬 수 있다. 사용 전 교육은 독성의 위험을 최소화 할 수 있도록 주의 깊게 이루어져야 한다. 2세 이하의 아이, 임신 중, 수유 중 또는 광범위한 피부염을 가진 환자에서는 금기이다.<ref name="Pharmacotherapy of ectoparasitic infections.">Roos TC, Alam M, Roos S, et al.   Lidane은 2세 이하, 임신 또는 수유 중, 광범위 피부염 환자에서 금기이다.

===치료 중 의복 및 침구류 관리===

*치료시작 시 의복과 침구류는 사면발이가 감염되지 않은 새것을 사용한다.

*의복 및 침구류와 같은 매개물은 뜨거운 물 (50℃)로 세탁하거나 드라이클리닝을 해야 하며 세탁물의 온도가 50℃ 이하인 경우 완전한 이의 박멸을 보장할 수 없다.<ref name="Efficacy of machine laundering to eradicate head lice: recommendations to decontaminate washable clothes, linens, and fomites.">Izri A, Chosidow O. *침대이불은 진공청소를 한다.

  의복 및 침구류는 50℃ 이상의 뜨거운 물로 세탁하거나 드라이클리닝을 한다.

===임신 또는 수유부의 사면발이감염===

 C   임신 또는 수유부의 사면발이감염은 Permethrin을 사용한다. 

===속눈썹에 발생한 사면발이감염===

*속눈썹의 이에 의한 감염증은 pyrethrin이나 lindane으로 치료해서는 안된다.<ref name="Scabies and pediculosis pubis: an update of treatment regimens and general review.">Wendel K, Rompalo A. Scabies and pediculosis pubis: an update of treatment regimens and general review. Clin Infect Dis 2002;35(Suppl 2):S146-S151. </ref>

*족집게를 이용하여 속눈썹에 들러 붙은 이를 제거하거나 폐쇄성 안연고 (occlusive ophthalmic ointment)를 눈꺼풀 가장자리에 10일 동안 1일 2회 바른다.

*또는 면봉을 이용하여 속눈썹에 permethrin 1% 로션을 바르고 10분 후 씻어 낸다. 이 때 permethrin은 눈자극은 없으나 눈을 감은상태에서 치료를 시행한다.
