외음부 및 회음부에 발생하는 acute fulminant necrotizing fascitis

=== 역학 ===
F<M, 60-80세 남성 (m/c)

사진은 기저질환으로 DM을 앓고 있는 환자가 penile prosthesis를 삽입한 후 발생한 Fournier’s gangrene으로 우측은 prosthesis removal 및 pus drainage, debridement 후 호전된 사진이다.

=== 위험인자 ===
DM, alcoholism, leukemia, immunodeficiency, drug abuser, surgery (rare)

=== 원인균 ===
polymicrobial (aerobic+anaerobic)

E. coli, Klebsiella, Proteus, Staphylococcus, Streptococcus, Bacteroides가 주된 원인균

=== 증상 ===
- Crepitant skin, dead & discolored tissue, Fever, drowsiness, incresing genital pain & redness (erythema), Odor, tenderness, penile and scrotal swelling

=== 진단 ===
- P/Ex. (악취를 동반한 급속히 진행하는 vesicle를 동반한 necrotizing swelling)

=== 치료 ===
(1) 적극적인 debridement → initial debridement의 효과가 의심스러운 경우 24-48시간 이후 secondary debridement 시행

(2) debridement 전 충분한 IV hydration 및 항생제는 초기에 metronidazole을 포함한 삼제요법을 시행해야 함.

(3) orchiectomy는 대부분 불필요; testis는 fascia 및 scrotal blood supply와 독립된 blood supply를 가지기 때문

(4) suprapubic diversion (urethral trauma or extravasation의 경우)

(5) colostomy (colonic or rectal perforation의 경우)
