Varicocelectomy

Check list

	II-1	Skin incision on external ring

	II-3	Identify external inguinal ring and spermatic cord
Level III		Spermatic cord isolation
	III-1	Grasp the cord with Babcock clamp and encircle with Penrose drain
	III-2	Tie dilated posterior cremasteric vein
	III-3	Open external spermatic fascia
Level IV		Ligation of spermatic vein
	IV-1	Use Optical loupe and microscope set with magnification, identify the spermatic veins
	IV-2	Use papaverine hydrochloride to relieve vascular spasm
	IV-3	Tie the spermatic vein with 3-0 silk
Level V		Closure
	V-1	Close Scarpa`s fascia
	V-2	Close Skin

Subinguinal varicocelectomy

• Patient position : Supine and in slight reverse Trendelenburg position

1. External ring에서 Langer’s line을 따라 3cm transverse incision
2. Subcutaneous layer를 지나 Scarpa fascia까지 도착
3. Index finger를 이용해 Scarpa fascia를 blunt dissection 하고 subinguinal spermatic cord를 관찰
4. Babcock clamp를 이용해 cord를 들어올리고 Kittner dissector를 이용해 cremasteric m.과 cord를 분리
5. Perforating external spermatic vein을 4-0 silk로 tie하고 절제
6. Testis를 절개창 밖으로 꺼내어 확장된 gubernacular vein을 4-0 silk로 tie하고 절제하고 testis를 scrotum 안으로 위치 시킴
7. 설압자를 spermatic cord 밑으로 넣고 external spermatic fascia를 절개
8. Gonadal artery의 pulsation을 확인하고 gonadal artery와 vas deferens, lymphatics를 보존하고 dilated spermatic vein을 4-0 silk로 tie하고 절제
9. Scarpa fascia와 subcutaneous layer 그리고 skin을 layer to layer로 봉합하고 수술을 마침

Inguinal varicocelectomy

• Patient position : Supine and in slight reverse Trendelenburg position

1. Symphysis pubis에서 2finger 위 정도 level에서 scrotum의 lateral aspect에서 시장해 inguinal canal쪽으로 oblique하게 3cm 크기로 절개
2. Subcutaneous layer를 지나 scarpa fascia까지 도착
3. Scarpa fascia를 절개하고 external oblique aponeurosis를 절개하여 inguinal spermatic fascia를 노출 시킴
4. Spermatic cord를 thumb과 forefinger를 이용해 들어 올리고 설압자를 cord 밑으로 넣음
5. Spermatic fascia를 절개하고 vas deferens와 artery를 촉지하여 분리 시킴
6. Artery와 vas deferens, lymphatics를 보존하고 dilated spermatic vein을 4-0 silk로 tie하고 절제
7. External oblique aponeurosis를 4-0 synthetic absorbable material로 suture
8. Scarpa fascia와 subcutaneous layer 그리고 skin을 layer to layer로 봉합하고 수술을 마침
