attack Hx, NPO time · P.I: 환자 평소 (STD의 과거력 있던 자, dirty sexual contact history있던 자, trauma Hx 있던 자, 건강했던 자) 로 부터 갑자기 develop된 pain을 주소로 본원 ER내원 · PHx: Hepatitis/HTN/DM/Tbc (///) OP Hx STD, dirty sexual contact history Hx, Trauma Hx · ROS: A/N/V/C/D (////), F/C (/) urinary Sx: dysuria (-/+), tenesmus (-/+), scrotal pain · P/Ex: Vital, Mental, General condition Head & Neck: anemic, icteric, soft neck Chest: Heart, lung sound Abdomen: Bowel Sound, abd scar, abd T/RT (/), CVAT (/) Extrimities: pitting edema, grossly free Scrotal examination: tenderness, swelling, mass palpable testis, palpable epididymis · Prehn’s sign

(if pain → Tarasyn 1@/#od IM · Lab.: 1. urine study: U/A with microscopy 2. serum study: BUN/Cr, CBC, electrolyte · Plan & recommend 1. 입원장 줄 것. 2. 바로 scrotal sono (Color doppler) ①. Plan: if epididymitis 3. IV antibiotics start 4. scrotal elevation & cold bag. 5. prn inj. (1% lidocain 20cc를 고환 상부 정계에 주입 한다.) 6. 만일 증상이 심각하지 않거나, 환자가 자의 퇴원 시엔 D/C c medication (Cravit 3T/#3, Beszyme 3T/#3, Soleton 3T/#3) * 7일 GU OPD F/U ②. Plan: if scortal torsion 3. Manual detorsion (Rt:시계 반대방향으로, Lt:시계 방향으로 풀어준다. → 외회전방향) 4. Emergency Pre-op Lab, 필요시 shaving 5. 마취과 연락. 6. Get Op permission. ③. Plan: if testicular rupture 3. Emergency Pre-op Lab, shaving 4. 마취과 연락 5. Get Op permission # epididymitis와 torsion 감별 위해 필요하면 testicular scan (핵의학검사)을 시행할 수 있다 (요즘 거의 시행 안함.).
